If you are unable to complete the online re-enrollment in one sitting, your snapcode will save the information you have entered until you are able to complete the form. As part of the online re-enrollment, we ask you to complete an anonymous survey about your school. Results are submitted separately from your re-enrollment information. Your responses will allow us to better serve your family in the coming school year. <\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400\"> Re-enrollment begins the week of December 5.<\/span><\/li>\n<li>To complete the online re-enrollment form, parents will need to:<\/li>\n<\/ol>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Update emergency contact information<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Update any changed parent\u2019s contact information (email, phone number, address). Please contact your school office to update your information.<\/span><\/li>\n<\/ul>\n<ol start=\"3\">\n<li><span style=\"font-weight: 400\">If your student\u2019s immunization record has changed, please submit to the school an updated record properly signed by a doctor or clinic staff before the second month of the 2017-18 school year.<\/span><\/li>\n<\/ol>\n<p><b>The following documents require a Parent\/Guardian signature:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">At Risk Survey<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">School copy of\u00a0<\/span><a href=\"http:\/\/responsiveed.com\/about\/information\/handbook\"><span style=\"font-weight: 400\">Parent\/Student Handbook Receipt acknowledgement form<\/span><\/a><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Voluntary Photo\/Video Release<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Notice of Compulsory Attendance Law<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If residency has changed: two proofs of residency (utility bill, lease, rent receipts, driver\u2019s license, etc.) Parent or guardian\u2019s driver\u2019s license is preferred, but optional.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Anonymous school survey <\/span><\/li>\n<\/ul>\n<hr \/>\n<p><b>Para estudiantes que regresan:<\/b><\/p>\n<p><span style=\"font-weight: 400\"><span class=\"dropcap dropcap3\">P<\/span>ara estudiantes que regresan, los padres tendr\u00e1n que completar el formulario de inscripci\u00f3n en l\u00ednea y entregar el registro de vacunas m\u00e1s reciente de su hijo.
