At the beginning of the 2016-2017 school year we made all of the forms for enrolled students available through Jupiter. Please allow a minimum of two school days for forms to be processed. Some forms will require additional time.
To access forms, please log in to Jupiter, then select “Menu,” “Downloads,” and then “School Office.” If you are having trouble logging on to Jupiter or finding your forms, please email [email protected]

Student Information and Agreements
This form is mandatory for all students. It allows you to update emergency contact information and tell us who is approved to pick up and drop off your child. It will also allow you to opt out of the student directory, if you wish, and decline to have your student’s photo used on the school website and other publications. This form also allows you to acknowledge that you have read and agree to the requirements outlined in the school handbooks, something that must be done before your child begins school with us. If you have multiple children in the school, please fill out one form per child.
Verification of Enrollment (VOE)
This form is required to obtain a driver’s permit and a driver’s license.
Parking Permit Request
This form allows your student to park on campus and must be renewed annually.
Transcript Request Form
College Visit Request Form
This form allows you to notify us that your child is going to visit a college and request that the absence from school be excused.

Free/Reduced Meal and Uniform Assistance Online Application
Please complete this form to qualify for free and reduced meals and the uniform assistance program.
